The test allows your copyright to be in there.  I've fixed it to
not require the Kitware copyright for initial contribution.  I also
tweaked the syntax in your notice to pass the (new) test:

I must insist on the ==== dividers.  It clearly isolates the
notices.

Our copyright is placed on the modifications we've made since
contribution.  All original copyright holder/year notices are
still present.  If any author failed to include a notice upon
initial contribution, he/she is free to submit a patch that
retroactively adds notice of his/her copyright for the proper
year range.
